I'm working on some code that fails to fetch from a cursor when it
commits a transaction after the cursor is opened, but before the fetch.
I've tried to make this work with "SET CURSOR_CLOSE_ON_COMMIT OFF" for
the connection, but it doesn't seem to do the trick. I can only make it
work by moving the commit to follow all of the fetches. While each query
is easy to fix, there are a lot of them and they are not always under my
control.

Am I missing something on the meaning of SET CURSOR_CLOSE_ON_COMMIT OFF
statement?

SET CURSOR_CLOSE_ON_COMMIT OFF is the correct.
if you try to close your cursor after the transaction is committed do
you get an error message: "Cursor is not open" ? If you do then
perhaps your SQL server is setup to create local cursors by default and
they are going out of scope when the transaction is committed. You can
check the 'default to local cursor' option using sp_dboption.
